New patient inquiries are closed until further notice so that current patients can have time to heal.  Please check back for updates. When new patient appointments are open you will be able to fill out the New patient Request Form and any other necessary information.  After submitting these you will be contacted to schedule your appointment. Please note: We are not able to bill OHP, Mediciad or Medicare policies. ​We do NOT submit insurance for patients from Trillium (Oregon Health Plan; OHP), Triwest/Tricare (VA), PacificSource Medicare Advantage plans, United Healthcare Medicare advantage plans, Providence Medicare Advantage plans, CIGNA, CHP Group, Kaiser, HealthNet, WellCare, Aetna, Providence (except Collective Health in 2026), or Workman's Comp.
